Dr. Shaun Murphy, a young surgeon with autism and savant syndrome, relocates from a quiet country life to join the prestigious St. Bonaventure hospital’s surgical unit. As his romantic relationship with Lea deepens, he will also face new responsibilities as a fourth-year resident when he is put in charge of supervising a new set of residents that will test him in ways he cannot predict. Meanwhile, the team must deal with the uncertainty and pressure that the COVID-19 pandemic brings now that it has hit their hospital.

Shaun and Lea's upcoming engagement party has everyone in a festive mood; a young single mother learns her son may have contracted his cancer from a surprising source; Mateo finds out if his previous issues in America will be resolved.

Shaun and Lea’s long-awaited wedding reception is interrupted by a violent attack at the hospital and the entire team must mobilize to save the victims. Meanwhile, the trauma of seeing people he cares for in life-threatening situations is affecting Shaun in more ways than one.

Life hasn't run out of curveballs for Dr. Glassman, either, and it seems determined to force him to relive one of the hardest lessons he's yet refused to learn. Not all is bleak, though, as Drs. Park and Reznick take stock of their relationship and a chance reunion between Drs. Kalu and former team member Dr. Claire Brown soon has them entangled again as though no time had passed.
